Education Background Form Instructions:
Applicants must provide the information requested below in full for every school attended in order to be considered for admission.
For each school attended provide:
- Number of the year in school. Your first year in school is number 1; the second year you were in school is number 2, and so on. You must account for every year. If you were out of school for a length of time, it must be noted. Allow one section for each year in school.
- Standard grade, form, class, sixieme, cinquime, sexta, or name of the class or level you attended that year.
- Your age when you began the year in school. If you were 6 years old when you attended school for the first time, put 6 in the "Your Age" box in the first section.
- Calendar year or years for the year in school. For example: 1992-1993.
- Full name of the school attended that year.
- Location of school attended that year. Write the city, village, or town where the school is located. Write the country if it is different than your country of citizenship.
- Kind of school you attended such as Kindergarten, Elementary, Grundschule, Volkschule, Mit- telschule, Gymnasium, Lycee, Collageo, Superieur, Secondary School, Grammar School, Teacher’s College, University, etc.
- Language used in the classroom by your teachers.
- Examination you passed or certificate or diploma you received at the end of each school year.
